When making an appointment online or in
person, please inform us of: a) your primary/secondary insurance
carrier, b) which practitioner you wish to see, and
c) the
reason for the visit or health problem you are most concerned about.
We appreciate receiving 24 hours notice if you are unable to keep your
appointment. This will allow us to give another patient the
opportunity to be seen at that time.
We encourage you to download or print off and
fill in the Initial Visit Form and the History Form prior to your
appointment to save you time at your visit.
If you wish to have these forms mailed to
your home before your visit, please ask us to mail you a packet.
Please bring the completed forms with you to your appointment. If
you have not received these forms or do not have these forms completed,
please arrive 15 minutes early for our appointment.
